Dry Eye Disease: Why Most Treatments Miss the Real Cause

The complaint we treat more than any other on Harley Street — why it is so often misread, and what genuinely brings relief.

We can almost set our watches by it. Somewhere in the late afternoon, a patient sits down, blinks, and tells us, usually with a small, apologetic shrug, that their eyes have "just been feeling a bit tired lately."

More often than not, within a minute of looking through the slit lamp, we already know what we are seeing: Dry Eye Disease.

It is comfortably one of the most common conditions we treat on Harley Street, yet it remains remarkably misunderstood. According to NHS guidance and data from Moorfields Eye Hospital, dry eye affects anywhere from 1 in 4 to 1 in 3 adults over the age of 50, though in our central London practice, we see it across every age group, from young corporate professionals to frequent travellers.

So let’s talk about it and address a few fascinating, clinical facts about dry eyes because most people live with gritty, irritated eyes for years longer than they ever need to.

1. The Two Types of Dry Eye (And Why Treatment Fails)

There is one sentence we hear almost every week: “I’ve tried every drop in the pharmacy, and none of them work.”

We always feel for patients when they say it, because the frustration is real; but it also tells us something straight away. It almost always means no one has yet diagnosed which type of dry eye they actually have.

Eye care specialists categorise dry eye into two primary types:

  • Type 1: Aqueous Deficient Dry Eye (Not Enough Tears) Your lacrimal glands simply don't produce enough of the watery component of tears. This accounts for only about 10–15% of cases and is often associated with autoimmune conditions, hormonal shifts, or certain medications.

  • Type 2: Evaporative Dry Eye (Tears Drying Out Too Fast) Your eyes produce plenty of water, but the tears evaporate almost instantly. This accounts for over 80% of dry eye cases and is caused by a blockage or dysfunction in the tiny meibomian glands lining your eyelids.

Reach for a standard watery drop off a chemist's shelf when you actually have Evaporative Dry Eye, and it’s like pouring water into a bucket with no lid — it evaporates straight away. Same symptom; opposite cause.

2. The Watering Paradox: Why Streaming Eyes Mean Dryness

This is the single most confusing fact about dry eyes, and we explain it several times a week.

If your eyes water really stream, especially when stepping outside into the London wind that is very often severe dryness, not the opposite of it.

The Medical Fact: When the surface of the eye becomes microscopic-dry, it sends a panic signal to your brain. Your body responds with reflex tearing, flooding the eye with emergency tears. However, these emergency tears are purely watery — they lack the protective lipid (oil) layer created by healthy meibomian glands. They roll straight down your cheek instead of staying on the eye, leaving the surface just as dry as before.

Treat the underlying lipid deficiency, and the streaming almost always calms down. Patients rarely believe us until it happens.

3. Why London is a Perfect Storm for Dry Eyes

Harley Street sits in the middle of one of the most screen-bound, high-intensity square miles in the country.

The College of Optometrists notes that environmental factors play a massive role in tear evaporation. If you set out to design an environment specifically to provoke dry eye, it would look a great deal like a typical London working day:

  • The Screen Freeze: When staring at laptops, phones, or monitors, our blink rate drops by up to 60%. Worse still, our blinks become "incomplete"; meaning the eyelids don't fully close to pump fresh oil across the eye.

  • Air Conditioning & Climate: Modern office HVAC systems constantly strip moisture from the air.

  • The Tube Network: The dry, warm air inside the London Underground accelerates tear evaporation before you even arrive at your desk.

None of this is anyone's fault — but it does mean a generic bottle of eyedrops rarely stands a chance on its own.

4. What We Are Actually Looking At in the Chair

When you book a dedicated Dry Eye Assessment at London Eyecare Collective, we are not just checking whether your eyes are dry. We are studying the complex structure of your tear film, an astonishingly thin layer of oil, water, and mucus that your eyelids sweep across the eye every time you blink.

Using high-magnification diagnostics, we:

  • Time your Tear Break-Up Time (TBUT): Measuring precisely how many seconds your tear film stays smooth before breaking up.

  • Evaluate Meibomian Glands: Checking whether the oil glands are clear, thickened, or dormant.

  • Stain the Surface: Checking for micro-abrasions or fine, dry patches on the cornea that over-the-counter drops will never mend.

Only then do we truly know what we are dealing with, and only then is it fair to suggest a bespoke treatment plan.

5. How We Restore Lasting Comfort

Here is what we almost never do: hand you a generic bottle and send you on your way.

For most people, establishing the right foundations changes everything:

  • Lipid-Based & Preservative-Free Drops: Carefully chosen to match your exact tear deficiency.

  • Thermal Meibomian Therapy: Targeted heat and lid hygiene to unblock lipid glands and get your natural oils flowing again.

  • Punctal Plugs: For persistent cases, we can insert microscopic, completely reversible plugs into the tear ducts. This stops your natural tears from draining away too quickly, keeping the eye naturally lubricated.

  • Ophthalmology Referral: On the rare occasion a case requires medical or surgical intervention beyond optometrist care, we work alongside London’s leading Harley Street ophthalmologists and Moorfields specialists to ensure seamless care.

For patients who prefer complete privacy and discretion, we also offer private practice bookings where the practice is dedicated exclusively to you for the duration of your appointment.

The Takeaway

Please don't spend another year quietly putting up with gritty, tired, or watering eyes because you've assumed "it's just from working on computers." It usually isn't, and you don't have to live with it.

Come in and let us look properly most people are genuinely taken aback by how comfortable their eyes are meant to feel.

Frequently Asked Questions

Can dry eye actually be cured?

For most people, it is beautifully managed rather than permanently "cured." However, once we treat the specific root cause (water vs. oil deficiency), lasting, daily comfort is the standard outcome.

Not at all — the right ones help enormously! The trouble is buying them blindly. Many over-the-counter drops contain preservatives that can actually irritate the eye with long-term use, or they lack the essential oil phase needed for evaporative dry eye.

This is known as reflex tearing. When the eye's surface dries out, it signals the brain for emergency lubrication, producing a flood of watery tears that lack protective oils. It is the single most common myth we debunk in clinic.

We set aside 45 minutes. Thorough diagnostic care cannot be rushed in 5 minutes between routine eye tests. We'd far rather do it once, properly.
